Rhinoplasty is a surgical procedure that reshapes and can improve the function of the nose. People seek rhinoplasty for a number of reasons, including cosmetic refinement, birth defects, or to correct breathing difficulties. A double board-certified specialist in rhinoplasty, Dr. Gross has more than 17 years of experience in facial plastic surgery. After graduating with honors from the University of Miami School of Medicine, he trained at Tufts and Boston University. Dr. Gross is on staff at Orlando Regional Medical Center and is now the President-elect of the FL Society of Facial Plastic Surgery. He is recognized as a rhinoplasty expert in Orlando and has been named “Best Plastic Surgeon” by Orlando Magazine readers.
Dr. Gross has a detailed understanding of nasal anatomy and function. His background in reconstructive facial surgery and additional board certification in Otolaryngology (ENT), uniquely qualifies him to perform Rhinoplasty. “Rhinoplasty is arguably the most challenging of all plastic surgery procedures, requiring 3 dimensional knowledge of anatomy, nasal function, and an artistic eye for beauty,” says Dr. Gross. ”There are many intrinsic rewards in performing nose surgery”, says Dr. Gross. ”We are changing people’s lives in a positive way by helping them regain their confidence and improve their appearance. He performs nose surgery to correct or improve the appearance of common conditions, including crooked nose, long nose, saddle nose, wide nose, ethnic nose, short nose, nasal blockages, sports injury, childhood injury of nose, nasal hump reduction, tip elevation, tip plasty, tip lifting and shortening, nostril narrowing, nose bridge narrowing, and breathing improvement. The most challenging and demanding portion of nose surgery is the tip. “Controlling and re-shaping the tip, or tip plasty, requires in-depth and detailed knowledge of nasal anatomy, tip dynamics, healing processes, ethnic variations, and physiologic function,” says Dr. Gross. “Proper treatment of the tip is the major determinant to a successful aesthetic outcome in cosmetic Rhinoplasty,” he adds. This is where experience and case volume is very important and Dr. Gross has the most experience in this area, bar none. Many respected doctors including; dermatologists, dentists, ophthalmologists, oral surgeons, ENT ‘s, & plastic surgeons, refer patients to Dr. Gross for his expertise in Rhinoplasty.
He also performs correction of deviated septum and nasal turbinate cautery, which improves nasal airway function. Nasal implants can improve the appearance of the nose, and cartilage grafts using either septal nose or conchal ear cartilage can augment or correct the shape of the nose. Dr. Gross routinely performs osteotomies to reset the nasal bones and bring in the sidewalls to reduce the wideness of the bridge and tip. Dr. Gross also provides revision rhinoplasty, a corrective procedure for a prior nose job or botched nose job.
Dr. Gross perfoms ethnic rhinoplasties as well, a procedure that refines the function and form of the nasal shape while preserving the distinct ethnic features of the nose. Ethnic nose variations include Asian, African-American, and Hispanic, which may need structural support. Caucasian noses typically require hump reduction or straightening and have good overall support. A popular procedure for ethnic rhinoplasty is nasal bridge elevation (or reduction) and nostril reshaping. Dr. Gross’ artistic judgment and understanding of ethnic facial variations are a key to his success in performing ethnic rhinoplasties.
Careful pre-op measurements, internal and external nasal exam, and computer images combined with Dr. Gross’ artistic eye are the foundation of balanced and natural rhinoplasty results. According to Dr. Gross, “a good rhinoplasty should whisper across the room….not shout”. Natural results, he notes, are key in rhinoplasty. He is described as a controlled and delicate surgeon, a gentle sculptor who uses new and advanced techniques to achieve refined yet noticeable results. Dr. Gross uses Open Approach Rhinoplasty for greater visualization and direct control of nasal tip. This technique is mandatory for Revision Rhinoplasty and provides more predictable outcomes which substantially improve the aesthetic look and function of the nose, according to Dr. Gross.

The rhinoplasty procedure is approximately 2 1/2 hr. procedure at Primera Surgery Center, our AAAASF accredited, on site surgery center. The same day surgery is an outpatient procedure, with general anesthesia provided. After anesthesia, an adult driver is needed to take the patient home, where they will experience an easy at-home recovery process. Ice compresses applied to the forehead for the first two to three days will minimize swelling. Arnica tablets are provided to reduce bruising and speed healing. Because there will be no packing in the nose, patients enjoy greater comfort during the recovery process. “Patients are really quite surprised that there is so little pain after surgery. A return visit will be necessary at one week post-op for cast and suture removal. Patients can return to school or work the day the cast is removed, looking quite normal and natural. Although the reveal is exciting, patients should expect their rhinoplasty results to continue improving over the first year as swelling gradually subsides. It is expected that there may be some temporary numbness at the tip of the nose.

How long does the Rhinoplasty ( Nose Surgery) take and what kind of anesthesia is used?
Dr. Gross performs Rhinoplasty in approximately two to three hours depending on the complexity and the condition of the nose. Rhinoplasty is done under general anesthesia at Primera Surgery Center as an outpatient surgery with at home recovery for one week.
How accurate is the Computer Imaging session?
Dr. Gross has over 15 years of experience with Computer Imaging and approximates his surgical outcomes very closely with those provided on the Imager. Barring any unforeseen problems, most patients will be very satisfied with the final results Dr. Gross simulates on the Computer Imager. Dr. Gross will examine each patient individually and advise each patient if their expectations are realistic, based on this comprehensive analysis. These images are emailed to the patient containing a standard Disclaimer.
Is there a lot of bruising afterwards and how long will the swelling last?
This depends again on how complex the procedure is, but in general most of our patients experience limited discoloration and swelling around the eyes and this is resolved within one week. This due to the experience and skill of our surgeon. “Delicate care in surgery and handling the tissues gently is critical for rapid healing”, says Dr. Gross. We also prescribe homeopathic Arnica and Bromelain (Sinecch) to reduce swelling and bruising.
I have trouble breathing through my nose; will cosmetic Rhinoplasty help my breathing?
Yes. The condition described is usually associated with a deviated septum or a bent internal framework of the nose. “Most of the time this results from trauma such as a broken nose, but many of my patients cannot recall an episode where this might have occurred,” says Dr. Gross. Careful examination by a qualified Facial Plastic Surgeon is needed to determine the best course of treatment for each patient. Dr. Gross is double board certified which allows for his unique skills combining cosmetic as well as functional improvement during surgery. Although correction of a cosmetic deformity of the nose is not covered by insurance; Dr. Gross will correct breathing problems, as needed, at the time cosmetic Rhinoplasty(Nose Surgery) is performed. Keep in mind that Dr. Gross is one of only a very few doctors with dual board certification and training to handle both the cosmetic and breathing portions of your surgery. Although we do not accept insurance; Dr. Gross may advise you on the benefits of correcting your breathing problems during surgery .
Where are the incisions placed during nose surgery?
Dr. Gross places most of the incisions for Rhinoplasty( Nose Job) hidden in the inside of the nostril. External incisions may be hidden in the creases where the nose meets the cheek to narrow the nose. An incision will also be hidden under the nasal tip for Open Rhinoplasty.
When will I see my final results from surgery?
It takes one year for all of the swelling to subside , especially at the tip and following tip plasty. This may be longer for revision Rhinoplasty patients or previous trauma cases, which we frequently care for. However, most patients see immediate pleasing results when the cast is removed at the end of the first week. The typical patient returns to work or school at the end of one week with very little signs of nose surgery.
Will the bump re-grow on my nose after my nose surgery?
No. The bump will not re-grow on your nose; however, we recommend our patients follow strict guidelines to avoid contact sports for one month following surgery. This will avoid any unnecessary trauma or injuries.
Will I have packing inside my nose?
No. Dr. Gross performs cosmetic Rhinoplasty with minimal invasiveness. His advanced techniques allow for minimal trauma and bleeding in surgery resulting in less pain and swelling; therefore, no packing is required. “Our patients experience minimal discomfort and bruising during their recovery” says Dr. Gross. Arnica and Bromelain (SinEcch™)are prescribed to our patients to minimize bruising and swelling- a homeopathic remedy of great benefit in the healing process.
What is tip plasty?
Any modification of the tip of the nose is considered tip plasty. Dr. Gross re-shapes, sutures, and trims the cartilage at the tip of the nose for a more refined tip, which is less round or bulbous. Tip plasty is an advanced form of technique in Rhinoplasty and requires a great deal of experience and training to even perform these maneuvers. Many plastic suregons don’t address the tip because of it’s complexity and challenges. However, Dr. Gross performs tip plasty routinely for more harmonious results.
