As Medicine has changed over the years, so too has the practice of Plastic Surgery. With reimbursements for primary care doctors declining, we are seeing increasing numbers of doctors claiming to be ‘cosmetic surgeons’ in an effort to escape the insurance-based reductions in their revenues. Weekend lipo courses, weekend facelift seminars, and even “Botox for Dummies” are attracting unqualified doctors with sometimes questionable training and backgrounds. After taking a crash course, they return to their offices with a shingle claiming to be trained to perform cosmetic procedures, but in fact have nothingmore than a medical license and a certificate to show. Or worse, cosmetic procedures such as Lifestyle lifts, have become commercialized and are mass marketed to the public as a one-size fits-all approach —touting “a facelift in about an hour”.

Facial Plastic Surgeons, General Plastic Surgeons, and Dermatologists are the widely recognized experts in performing cosmetic procedures. They have completed a dedicated 3-5 year residency program, passed a board certification exam, and received appropriate surgical training. To name a few specialties now offering body plastic surgery, cosmetic facial procedures, and Botox—dentists, gynecologists, pediatricians, family practitioners, and emergency room doctors. The public should research the credentials of those providers offering services outside their recognized field of expertise and question the training of doctors and dentists who attend ‘weekend courses’ in cosmetic procedures. “There are legitimate concerns for patient safety and poor outcomes when unlicensed providers [claiming plastic surgery qualifications] treat un-informed patients”, according to Dr. Gross.

We proudly display our seal of certification from the American Board of Facial Plastic & Reconstructive Surgery. To earn this distinction, Dr. Gross completed a 5-year residency in Head & Neck Surgery at Tufts and Boston University and was then granted a Fellowship in Facial Plastic & Reconstructive Surgery. He was then required to pass a two-day board certification process and has continued his work with the board as an examiner for new doctors, is past-president of the Seminole County Medical Society, and is President of the Florida Society of Facial Plastic Surgery. He has been featured in the Orlando Sentinelas leading facial plastic surgeon and lectures at local and national conferences.

Mini-lifts for both men and women are becoming more popular and are a great alternative to a larger surgery later on in life.

With so many advancements in facial plastic surgery over the past 20 years, it’s easy to see how many people can become confused about the difference between the traditional facelift and the more recent emergence of the minilift. A facelift should actually be called a lower facelift and necklift. This would give patients a clearer definition of what the actual procedure corrects. When the problems are confined to the jowls and neck, a lower facelift is exactly what is needed. A full facelift could include eyelid surgery and a browlift, and many patients do not need everything done. It used to be that patients would wait until they were older, and then go in for one big procedure. Now, many people opt for procedures earlier, as they begin to notice areas of concern. It’s basically a philosophy of maintenance, rather than renovation. The sooner you make a decision to start looking younger, the sooner you will look youthful and refreshed. Doing procedures individually at an earlier age, allows for smaller procedures, which means a faster recovery and less drastic changes.

As far as exactly when to do things, that’s based highly on the patient’s needs. A 40 year old would generally be considered a bit young for a traditional facelift, but it depends on what type of skin you have, how much sun damage you’ve had, and what your goals are.

A minilift is a good option for younger, healthy patients. A minilift involves a short incision around the ears, less healing time, and quicker recovery. This shorter procedure tightens the jaw line and lifts the neck too. It’s ideal for 35 to 50 year olds who want a more youthful face and neck without the downtime. It is becoming very popular for men and women. Best results are seen when lifting procedures are combined with volumizing injections such as Radiesse, Juvederm, or Sculptra. As the face ages, we see sagging, lines, bags, wrinkles and volume loss. I see the best results when patients have volume replacement in the cheeks and around the mouth following mini-lifts. Newer concepts in reversing aging signs are to treat these depleted areas with fillers to restore a more youthful look.

The American Academy of Facial Plastic and Reconstructive Surgery was founded in 1974 and is the world’s largest specialty society for the specialty of facial plastic surgery which represents over 2,700 facial plastic and reconstructive surgeons throughout the world. The AAFPRS is a National Medical Specialty Society of the American Medical Association (AMA), and holds an official seat in both the AMA House of Delegates and the American College of Surgeons board of governors. AAFPRS members are board certified surgeons whose focus is surgery of the face, head, and neck. AAFPRS members subscribe to a code of ethics.

Specialization in medicine has been one of the major enhancements in patient care over the last generation. AAFPRS members not only have a precise focus in patient care but they also have had more comprehensive training in facial surgery than any other medical specialty.

All AAFPRS members are board certified by a specialty board recognized by the American Board of Medical Specialties. A majority are certified by the American Board of Otolaryngology, which includes facial plastic surgery. Others are certified in plastic surgery, ophthalmology, and dermatology. A growing number of members are board certified by the American Board of Facial Plastic and Reconstructive Surgery.

In 1974, the Educational and Research Foundation for the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS Foundation) was created to foster, promote, support, augment, develop, and encourage investigative knowledge and charitable and humanitarian application of facial plastic and reconstructive surgery.

In the early 1990′s the AAFPRS Foundation’s humanitarian program was created.

Internationally, FACE TO FACE provides humanitarian assistance to primarily children suffering from facial deformities caused by birth or by trauma. Unlike some programs FACE TO FACE makes the education of facial plastic and reconstructive surgeons all over the world an integral part of its program. FACE TO FACE surgeons believe that a more lasting impact is made in communities when visiting and local surgeons and nurses work together in integrated teams to manage cases. Besides hands on training, all medical personnel participate in lectures and share published information.

Here at home FACE TO FACE: The National Domestic Violence Project helps individuals in our own backyard. Do you ever wonder what happens to the survivors of domestic violence after the abuser has traumatized their faces?Many of them receive severe facial injuries. Cigarette burns, broken noses, sunken cheekbones or gashes are painful daily reminders of the unwanted abuse. Due to financial reasons and/or feelings of low self-esteem they are not able to receive adequate medical care. Even though emotional scars will heal over time with the support of others, the physical ones do not fade on their own. Some live with their scars for the rest of their lives. These individuals, primarily women, are a forgotten segment of society.

AAFPRS Foundation was the first surgical group to take a firm stand and become involved in assisting survivors of domestic violence. AAFPRS surgeons offer consultation and surgery pro bono to survivors of this violence thereby enhancing their self-esteem and aiding them in rebuilding their lives.

We are pleased that in 2009 Faces of Honor joins our FACE TO FACE Programs. The purpose of Faces of Honor is to offer pro bono medical and surgical expertise to veterans who were deployed in Iraq or Afghanistan and received face or neck injuries. Our motivation is simple: provide a “thank you” to the brave men and women who have served our country so well by offering what we have worked hard to achieve, our surgical expertise and skills as caring physicians.

The Faces of Honor Program is coordinated by the American Academy of Facial Plastic and Reconstructive Surgery and its affiliated Foundation (together “the Academy”). The Academy’s role is to serve as a resource to help find surgeons willing to provide surgery to qualified veterans without a surgical fee to the patient. The Academy does not provide medical or surgical care. Consequently, the Academy does not enter into a physician/patient relationship with anyone. Not all Academy members perform all procedures included in the field of facial plastic and reconstructive surgery. The Academy makes no warranties or representations about the skills of individual physicians or surgeons or their abilities. A Face From an Infomercial

It used to be that a cosmetic surgery patient who was tired of sagging jowls and neck would discreetly ask for names of reputable doctors who did face-lifts. However, Face-lifts are now being marketed nationwide on TV or in infomercials from franchised clinics. Many doctors are concerned that patients may be so persuaded by infomercial advertising that they don’t seek a second opinion or investigate the franchise thoroughly. Have there been patient complaints or injuries, are the clinics properly licensed, are there other alternatives to a ‘one size fits all’ approach?

“Patients should be presented with a range of options during consultation and a ‘one size fits all’ approach to facial lifting is simply not a good idea. Every patient should have their surgical procedure tailored to their specific needs. Furthermore, “speed in a face lift procedure should never be a criteria to entice patients for surgery”, says Dr. Gross.

Here’s some advice for those considering face-lifts.

Check whether your surgeon is certified by one of the boards of the American Board of Medical Specialties at abms.org. They require physicians to complete residency training in a specialty and must pass rigorous oral and written exams.

Before you schedule your surgery, meet the surgeon to make sure you’re both on the same page. “The best surgeons will give you their honest opinion whether they can achieve your desired results,” says Dr. Gross.

Don’t settle for a hasty consultation without meeting the doctor. The downside as well as the upside of surgery should be covered.

You should never feel as if you’re being sold a procedure. “If you’re getting a high-pressure sales pitch for a procedure, personally that would worry me,” says Dr. Gross. It never hurts to get a second opinion.

“My dream has always been to have a comprehensive care center that can accommodate all aspects of patient care,” says Edward Gross, MD, who moved his successful practice to the Lake Mary location in 2006. “Seminole County residents can come here for spa treatments, minor cosmetic procedures and full face-lifts, all done within our fully accredited AAAASF surgical facility.”

“More men are coming in for procedures,” says Dr. Gross, “And we see more patients who want to take advantage of treatments that offer little or no downtime, like chemical peels, dermal fillers and Botox.”

“I am very focused on patient education, because I want my patients to make informed decisions,” Dr. Gross says.

Dr. Gross received his medical degree from the University of Miami School of Medicine and continued his surgical training in Boston at Tufts and Boston Universities. He was granted a fellowship through the American Academy of Facial Plastic and Reconstructive Surgery where his training was devoted to plastic surgery of the face, eyes, nose, head and neck.
